Responsibilities

The Assessment Nurse is responsible for conducting field and community-based clinical assessment visits. This role is responsible for evaluating prospective residents, completing required documentation, and determining their eligibility based on Company resident service guidelines. The Assessment Nurse also identifies individual service needs and collaborates with Regional and Care Management Teams to ensure continuity of care and support efficient, effective operations.

  • Perform assessments and reassessments to determine resident needs and establish appropriate service plans in collaboration with community based and regional team members
  • Manages assessment activities through the calendar, tracks the status of completion and communicates with the team.
  • Follows the assessment procedures which adheres to state, federal and company laws, regulations and guidelines.
  • Conduct comprehensive function assessments, including a neurocognitive examination upon pre-admission of residents or return from hospitalization/rehab stay.
  • Complete daily Quality Enhancement tasks to ascertain if the department is meeting state regulations and Atria policies.
  • Plans, coordinates, and completes the timely and comprehensive review of resident needs with members of the interdisciplinary teams such as nursing, social work, dietary, activities.
  • Adapt to changing schedules and community needs.
  • Manage communication seamlessly across a variety of platforms to keep community and regional leadership informed.
  • Compose and deliver professional verbal and written communication.
  • Collaborate with outside professionals to collect necessary medical support documentation.
  • Drive personal vehicle and/or company vehicle to conduct prospective resident assessments and/or reassessments across the assigned geographic area.  
  • May perform other duties as needed and/or assigned.

Qualifications

  • A registered nurse licensed to practice as required by State regulations.
  • Experience in direct resident care and assessments/observation of clinical
  • Conditions.
  • Experience in Assisted Living or a related industry experience preferred.
  • Excellent verbal and written communication skills, with the ability to convey information professionally.
  • Must complete all state required training to maintain licensure or certification.
  • Ability to work flexible hours to accommodate the needs of residents and their responsible parties. Frequent travel will be required on behalf of Company.
  • Position requires driving responsibilities (may use Company provided vehicle and/or personal vehicle).
  • Must possess valid driver’s license.
  • Must satisfactorily meet and be in compliance with Atria Motor Vehicle Policy standards.
  • Must successfully complete all Atria-specified training programs.

Salary Range
